How Do I Apply for the National Disability Insurance Scheme (NDIS)?
The NDIS has an initial Eligibility Checklist. Click on this link to start the process and see if you potentially qualify.
You can make an access request by calling the NDIS directly on 1800 800 110 or completing the Access Request Form and emailing it to NAT@ndis.gov.au.

How Do I Want My Funding Set Up?
There are three ways your funding can be set up:
Do you want your funding Plan Managed?
If you decide you want your funding to be plan managed, ensure you ask for this at your planning meeting. The funding category for this is called Improved Life Choices (CB Choice & Control).
Do You Need Support Coordination?
You might want to consider requesting Support Coordination funding.
How Can a Support Coordinator Help Me?
A Support Coordinator will support you to understand and implement the funded supports in your plan and link you to community, mainstream and other government services. A Support Coordinator will focus on supporting you to build skills and direct your life as well as connect you to providers.
Your Support Coordinator will assist you to negotiate with providers about what they will offer you and how much it will cost out of your plan. Support coordinators will ensure service agreements and service bookings are completed. They will help build your ability to exercise choice and control, to coordinate supports and access your local community.
They can also assist you in planning ahead to prepare for your plan review.
Support coordinators will assist you to ‘optimise’ your plan ensuring that you are getting the most out of your funded supports.
Other Important Things to Discuss at Your Planning Meeting
Although funding under your NDIS plan is largely flexible, there are certain services that are deemed ‘stated items’ under the NDIS. This means that you can only access these using your funding if they have been specifically approved and written into your plan. It’s important to request these at your planning meeting if you require these services. A quote for these services is then provided to the NDIS for approval and the funding is then made available to use. They include:
